Developmental Screening

Click on your child’s age to access the appropriate screening tool for your upcoming well child visit.  Please print SWYC and MCHAT forms, complete beforehand, and bring in to visit for review.

Birth to 2 months  (2week SWYC)

9 months (SWYC 9 month)

18 months (SWYC 18 month and MCHAT )

The Ages and Stages Questionnaires assess your child’s development within five core areas: communication, gross and fine motor skills, problem solving, and personal-social.

The M-CHAT is a tool we will include in Well Child Visits at 18 and 30 months of age developed to identify autism spectrum disorders and general developmental delays.

The Survey of Wellbeing of Young Children (SWYC) is a comprehensive screening instrument for children under 5 years of age.  This tool evaluates developmental milestones, behavioral/emotional development, and family risk factors.

Appropriate plans of care and/or referrals may be made based on the information gathered in these screening tools, or if risk factors or concerns are identified.
